  • Educator Licensure Graduate Programs

    Verification of Acceptance into Graduate Educator Licensure Program form

    This document is required for formal admission into the licensure portion of your program, and should be completed with your program coordinator at your plan of study advising appointment. Because the Commonwealth of Massachusetts may change educator licensure requirements, you are advised to contact the Department of Elementary and Secondary Education for specific, current licensure requirements. The Office of Licensure and Field Placement is also available to help you with licensure questions. You should meet regularly with your program coordinator and maintain the above documents to ensure that you will to meet the academic and licensure components of your program.

     

    Pre-Practicum Documentation form (PDF)

    Licensure programs culminate in a full-time, full-semester student teaching practicum or practicum appropriate to your program and license sought. Before your final practicum, you will need to complete all of the required courses which include pre-practicum field hours. Use these forms to document your pre-practicum field hours. Because these documents must be submitted as part of your practicum application toward the end of your program, it is critical you plan your pre-practicum field experiences with your program coordinator.

  • Master of Social Work

    Let the School of Social Work know you plan to matriculate
    Fill out and return the MSW Admissions Response Form in order to formalize your status as a matriculated student. Complete and return this form by the due date stated in your acceptance letter, along with your $200 non-refundable deposit (check or money order made payable to Salem State University) to the following address: Admissions Application Center, Attn: Brenda Vitale, Salem State University, 352 Lafayette St. Salem, MA 01970

     

    Attend MSW Program Advising and Orientation Events
    MSW advising and orientation day is mandatory for all newly admitted students -- both those in the regular and advanced standing program.

    • The required advising and orientation day is Tuesday, May 16, 2017 from 1-5 pm.
    • The Advanced Standing Field Orientation is Tuesday, May 16, 2017 from 8:30 am-12:30 pm. Advanced Standing Field Education Orientation is ONLY for Advanced Standing students in the full-time program and for those students who are beginning field education placement in May 2017.

    You will receive important information at this event that will help you plan for entering the MSW Program, including information on course scheduling, registration, and your plan of study. All students will complete an individual plan of study which outlines the required courses, elective, and which courses you will take and when. Please note that attendance at the Advising & Orientation Day event is required. Further details and directions will be provided in the spring.

     

    Attend Field Orientation and Practicum if you are a full-time student
    For full-time advanced standing students who will begin a field placement the week of Wednesday, May 24, your field orientation is Tuesday, May 16 from 8:30 am to 12:30 pm.

    For full-time students in our 62 credit MSW program who will begin a field placement in September 2017, a mandatory field orientation will take place Tuesday, August 29 from 8:30 am to 2:30 pm.

    Additional information for both events will come from the School of Social Work, and the field department will contact you in advance to discuss internship opportunities. 

  • Occupational Therapy

    Attend Graduate Orientation

    Direct Entry/MSOT Orientation will be held the first week in August. All newly accepted graduate students are expected to attend. Details will be sent to you from the OT department by mail.

     

    Submit immunization records and physical exam to the Office for Student Health

    • CPR certification for the Health Care Provider (A copy of current card must be submitted)
    • Liability insurance, purchased at the Bursar’s Office, Student Navigation Center, Central Campus Copy of receipt must be submitted to Sandy Krushenick
    • Proof of current Health Insurance, copy of card
    • HIPPA Certification (these are not needed upon admit, they will need this once they begin their fieldworks which is a year and a half into their program)
    • AOTA Membership card: membership in the American Occupational Therapy Association is required (A copy of the card must be submitted)
